06.12.21 SKC-ATX SKC Ball Retrieval Violation 96min
A ball was delivered by a Ball Retriever directly to a Player. This is the second violation of the 2021 MLS Season for the Club. The Club will be fined for this violation. Each violation thereafter will result in the doubling of the fine.

The audio recording device was without power and inoperable upon post-match inspection. The Match Director informed home Club Operations, and they said they were still in the process of replacing the unit. Noted to MLS Operations.
06.18.21 RSL-VAN
During the Match Coordination meeting, the Match Director asked the clubs and Match Officials about substitution cards. Vancouver and the Match Officials indicated that they did not have any. When Vancouver made their first substitution in the 61st minute, they handed the 4th Official a pink substitution card and indicated that it was the only substitution card they had in their possession. Home Club Ops rectified the issue after the first substitute issue was experienced. Noted to MLS Operations.

VMD was not present at the Match Coordination Meeting. Noted to MLS Medical.

The start of both halves was delayed due to broadcast. Noted to MLS Broadcast & Operations.

No Audio Recording Device at the 4th Official’s Table. Match Director was informed that it was in the Match Official’s Locker Room. Noted to MLS Operations.
The field was reportedly “cutting up” in several sections and posed a risk to player safety. Noted to MLS Operations.
WiFi connection is on a public access network which results in several workstreams (email communication, DMR management, VMD video) being impacted. Noted to MLS Operations.
